畸变产物耳声发射幅值与超高频纯音听阈的关系

摘 要:目的探讨畸变产物耳声发射(distortion
products otoacoustic emmission, DPOAE)与超高频纯音听阈用于早期耳蜗性听力损害的听力监测的可能性。方法DPOAE以配对初始音诱发,两音强度均为70dBSPL,频率比为1∶1.2,频率范围0.5~8kHz。测定常规频率(125Hz~8kHz)及超高频率(9~20kHz)的纯音听阈。比较正常听阈组与不正常听阈组的DPOAE振幅的改变,对DPOAE幅值与纯音听阈进行相关分析和直线回归分析。结果受试者中,近半数耳存在超高频听阈升高。不正常听阈组DPOAE的高频段(3~6kHz)幅值显著低于正常听阈组(P<0.01)。DPOAE高频段平均幅值与11.2、12.5、14、16、18kHz的超高频听阈直线相关(r=0.601,P<0.01)。结论DPOAE的幅值与超高频纯音听阈具有一定的相关性,两者均可能用于监测早期的耳蜗性听力损害。
